Ospamox - Pharmacological action
Ospamox is a beta-lactam antibiotic of the penicillin series with a pronounced bactericidal effect. The active substance of the drug is amoxicillin, a semisynthetic drug of the aminopenicillin group. The drug shows antimicrobial activity against Gram-positive and Gram-negative aerobes, as well as some strains of anaerobic bacteria.
Like other unprotected antimicrobial agents of the penicillin series, amoxicillin is inactivated by the action of penicillinase and is ineffective in treating diseases caused by strains of microorganisms producing beta-lactamases.
The mechanism of action of the drug lies in its ability to disrupt the synthesis of the biopolymer peptidoglycan, which is the main component of the cell membrane of microorganisms. Blocking the synthesis of peptidoglycan occurs due to the influence of the molecule of amoxicillin on penicillin-binding proteins, which are the enzymes necessary for the final stage of the synthesis of peptidoglycan. Thus, the drug Ospamox promotes the destruction of the structure of the cell membrane during the reproduction of bacteria, which ultimately causes their death.
Due to the lack of peptidoglycan and penicillin-binding proteins in mammals, the drug does not have a specific toxic effect on the macroorganism.
Ospamox is a broad-spectrum antibiotic that exhibits bactericidal activity against the following bacteria:
Gram-positive aerobic bacteria: Staphylococcus spp., Including Staphylococcus aureus and Staphylococcus haemolyticus (other than strains producing beta-lactamase), Streptococcus spp. (including Streptococcus pneumoniae).
Gram-negative aerobic bacteria: Corynebacterium spp., Campilobacter spp., Haemophilus influenzae, Proteus mirabilis, Neisseria meningitidis, Actinomycetes spp., Bordetella pertussis, Pasteurella multocida, Spirillum minus, Klebsiella spp., Bacillus anthracis, Escherichia coli, Shigella spp., Listeria spp ., Erysipelothrix rhysiopathiae, Spirochaeta (Leptospira, Treponema, Borrelia) and Streptobacilli spp.

Strains Streptococcus viridans, Neisseria gonorrhoeae, Salmonella spp., Bacteroides melaninogenicus and Fusobacterium spp. have a non-permanent sensitivity to amoxicillin, therefore it is recommended to perform sensitivity tests before starting therapy with Ospamox.
After oral administration, the drug is rapidly absorbed in the small intestine, the absolute bioavailability of amoxicillin for oral administration is 75 to 90%, depending on the dose taken. When using the drug in doses from 250 mg to 750 mg, the bioavailability is proportional to the dose, when using higher doses of the drug, the bioavailability is somewhat reduced. The intake of food at the rate of absorption and bioavailability of the drug is not affected. The peak concentration of active substance in the blood plasma is achieved after 1-2 hours after oral administration of the drug. The degree of bond of amoxicillin with plasma proteins does not exceed 17%.
The drug creates high therapeutic concentrations in virtually all biological fluids and body tissues, in particular high concentrations of amoxicillin are noted in the lungs, bronchial secretions, middle ear fluid, bile and urine. The drug penetrates the hematoplacental barrier and creates high concentrations in the amniotic fluid and fetal tissues, in an insignificant amount penetrates into breast milk. Ospamox is partially metabolized with the formation of pharmacologically inactive penicillic acid. Ospamox - Indications for use
The drug is used to treat acute and chronic infectious diseases caused by microorganisms sensitive to amoxicillin, including:
• Infectious diseases of the genitourinary system and gynecological infections: prostatitis , cystitis , urethritis, gonorrhea , septic abortion, adnexitis , endometritis , as well as acute and chronic pyelonephritis. The drug can also be used to treat asymptomatic bacteriuria during pregnancy.
• Infectious diseases of the respiratory system and ENT organs: acute and chronic bronchitis, bacterial pneumonia, lung abscess, acute otitis media, sinusitis, pharyngitis, laryngitis.
• The drug is also used to treat patients with infectious diseases of the skin and soft tissues, including secondarily infected dermatoses, erysipelas .
• Ospamox can be used as a prophylactic for surgical interventions in the oral cavity or upper respiratory tract.
In addition, after determining the pathogen, it is possible to use the drug as a monotherapy or in combination with other antimicrobial agents for the treatment of infective endocarditis and bacterial meningitis.
Ospamox - Dosage and administration
The drug is taken orally, the tablet is swallowed, washed down with water, if necessary, the tablet can be ground. It is also possible to take the drug in the form of a suspension, which is formed by dissolving the tablet in water.
Ospamox DT is taken regardless of food intake, it is recommended to take the drug at regular intervals. The duration of the course of treatment and the dose of the drug is determined by the attending physician individually.
Adults and adolescents over the age of 12 years with acute bacterial infections of different locations are usually prescribed 500-750 mg of the drug 2 times a day or 500 mg of the drug 3 times a day. In patients with severe forms of infectious diseases, the dose of the drug may be increased. The maximum daily dose of amoxicillin for adults is 3 g.
Adults and adolescents over the age of 12 years in the scheme of eradication Helicobacter pylori usually prescribed 1 g of the drug Ospamox DT 2 times a day.

The duration of the course of treatment is usually from 5 to 7 days, but in the presence of streptococcal infections it is recommended to prolong the course of taking the drug not less than 10 days. Patients suffering from renal insufficiency need a dose adjustment:
- Patients with a creatinine clearance greater than 30ml / min are prescribed in usual doses.
- Patients with a creatinine clearance of 10 to 30 ml / min are usually prescribed 0.5 g of amoxicillin twice a day at regular intervals.
- Patients with creatinine clearance less than 10ml / min are usually prescribed 0.5 g amoxicillin 1 time per day.

Ospamox - Side effects
The drug is usually well tolerated by patients, but in rare cases, the development of such side effects with the use of Ospamox:
• On the part of the gastrointestinal tract and liver: nausea, vomiting, anorexia , stool, dry mouth, flatulence, transient increase in hepatic enzyme activity. In isolated cases, there was a violation of taste sensations and staining of the tongue in a dark color.

Ospamox - Contraindications
Increased individual sensitivity to the components of the drug, as well as other beta-lactam antibiotics.

Phenylbutazone, oxyphenbutazone, indomethacin, sulfinperazone and acetylsalicylic acid in combination with the Ospamox DT drug increase the plasma concentrations of amoxicillin.
Antimicrobials that have a bacteriostatic effect significantly reduce the bactericidal action of amoxicillin.

Antacids in combination with the drug Ospamox DT reduce the absorption of amoxicillin.

Ospamox - Overdose
With the use of overestimated doses of the drug, patients may develop nausea, vomiting, diarrhea, and disturbance of the water-salt balance. With a further increase in the dose, convulsions may develop.
