Amoxisar - Pharmacological action
Amoxisar is a broad-spectrum antibiotic from the group of semisynthetic penicillins. It is bactericidal. By the mechanism of action refers to inhibitors of the synthesis of the bacterial cell wall.
It is active against gram-positive microorganisms: Staphylococcus spp. (not forming penicillinase), Streptococcus spp. (including Str.pyogenes, Str. pneumoniae, Str.viridans), most strains of Enterococcus faecalis, Bacillus anthracis, Clostridium spp., Corynebacterium spp .; of the majority of Gram-negative microorganisms: Neisseria gonorrhoeae, Neisseria meningitidis, Haemophilus influenzae, Moraxella catarrhalis, Escherichia coli, Salmonella spp., Shigella spp., Proteus mirabilis, Helicobacter pylori, Campylobacter spp., Prevotella melaninogenica.
The drug is resistant Proteus spp. (indole-positive), Moraxella morganii, Pseudomonas spp., Serratia spp., Enterobacter spp., as well as rickettsia, mycoplasmas, viruses.
Amoxisar is destroyed by penicillinase, therefore it is not active against penicillinase-forming strains of pathogens.
Amoxisar - Indications for use

- infections of the urogenital system (pyelitis, pyelonephritis, cystitis , urethritis, prostatitis);
- exacerbation of peptic ulcer of the stomach and duodenum, associated with Helicobacter pylori (as part of complex therapy);

In severe infection, these doses for adults and children can be increased by a factor of 1.5-2. In this case, the dose is administered iv. The daily dose is divided into 2-3 injections with an interval of 8-12 h
For prophylaxis of postoperative complications, the drug is administered IV for 30 minutes or IM for 1 hour before the operation at a dose of 0.5-1 g. Repeatedly during the first day of the postoperative period, the drug is used in the same dose every 8-12 hours. infected areas, the duration of preventive use can be increased to 3-5 days.
If the kidney function is impaired, you should reduce the dose of the drug or increase the interval between the administrations (24-48 hours). With SC from 10 to 30 ml / min the drug is administered in a daily dose of 1 g, then 500 mg every 12 hours; with CC less than 10 ml / min - 1 g, then 500 mg every 24 hours. In patients with anuria, the daily dose should not exceed 2 g. Patients on hemodialysis are prescribed an additional 1 g of the drug: 500 mg during the hemodialysis session and 500 mg after its termination.
Rules for the preparation and administration of a solution for intravenous and / or intravenous administration
For the / m introduction, the solution is prepared before administration, adding to the contents of the vial (1 g) 3 ml of water for injection.
For intravenous administration of a single dose of the drug (not more than 2 g) is dissolved in 10-20 ml of 0.9% sodium chloride solution and injected slowly for 3-5 minutes. With a single dose exceeding 2 g, the drug is administered intravenously drip.
For IV injection, a single dose (2-4 g) is dissolved in 125-250 ml of 0.9% sodium chloride solution or 5-10% dextrose solution and injected at a speed of 60-80 drops / min. With dropwise administration, a 5% dextrose solution (50-100 ml, depending on the age) is used as a solvent for children and administered at a rate of 10 cap / min. Amoxisar - Side effect
• Allergic reactions: urticaria , skin hyperemia, erythema , angioedema, rhinitis, conjunctivitis; rarely - fever, arthralgia, eosinophilia , exfoliative dermatitis, erythema multiforme, Stevens-Johnson syndrome ; reactions similar to serum sickness; in some cases - anaphylactic shock .
• On the part of the digestive system: dysbacteriosis, taste change, nausea, vomiting, diarrhea, stomatitis, glossitis, impaired liver function, moderate increase in hepatic transaminase activity, pseudomembranous enterocolitis.
• From the central nervous system and peripheral nervous system: agitation, anxiety, insomnia, ataxia , confusion, behavior change, depression , peripheral neuropathy, headache , dizziness , convulsive reactions.
• From the side of the urinary system: interstitial nephritis.
Other: shortness of breath, tachycardia , interstitial nephritis, vaginal candidiasis, superinfection (especially in patients with chronic diseases and reduced resistance of the body).
Amoxisar - Contraindications
- pronounced violations of the liver function;
- hypersensitivity to the drug or other antibiotics of the penicillin and cephalosporins.
With caution should appoint Amoxisar to patients with polyvalent allergies, gastrointestinal diseases (especially pseudomembranous colitis associated with the use of antibiotics), with renal failure.
Amoxisar during pregnancy and lactation
Caution should be used during pregnancy and lactation.
When using Amoxisar, regular monitoring of the function of the hematopoiesis system, liver and kidneys is necessary.
When using the drug, it is possible to develop superinfection by increasing the insensitive microflora, which requires a change in antibiotic therapy.
In the treatment of patients with bacteremia, the development of a bacteriolysis reaction (the Yarisch-Gerxheimer reaction) is possible.
There is cross-resistance and cross-over hypersensitivity to penicillin series drugs, cephalosporins.
In the treatment of mild diarrhea, which appeared on the background of the use of Amoxisar, the use of kaolin or attapulgite should be avoided by prescribing antidiarrheal drugs that reduce intestinal peristalsis. The appearance of severe diarrhea is the basis for stopping the use of the drug. Treatment should continue for another 48-72 hours after the disappearance of clinical signs of the disease.
With the simultaneous use of estrogen-containing oral contraceptives and Amoxisar, additional contraceptive methods should be used.
Amoxisar - Overdose
Symptoms: neurotoxic reactions, thrombocytopenia (with prolonged use in high doses).
These phenomena are reversible and disappear after the drug is discontinued.
Amoxisar - Drug Interactions
With the simultaneous use of Amoxisar with antibiotics that have a bactericidal effect (including aminoglycosides, cephalosporins, cycloserine, vancomycin, rifampicin) synergism of interaction is observed, and antibiotics that exert bacteriostatic action (tetracyclines, macrolides, chloramphenicol, lincosamides, sulfonamides) antagonism .
Amoxisar with simultaneous application improves the effectiveness of indirect anticoagulants (due to the suppression of the intestinal microflora, it reduces the synthesis of vitamin K and prothrombin index).
Amoxisar reduces the effectiveness of estrogen-containing oral contraceptives, which is accompanied by a risk of developing breakthrough bleeding.
Amoxisar with simultaneous use reduces the effectiveness of drugs, in the process of metabolism which forms PABC.
Diuretics, allopurinol, phenylbutazone, oxyphenbutazone, NSAIDs, drugs that block tubular secretion, while using with Amoxisar reduce its tubular secretion and increase the concentration of amoxicillin in the blood plasma.
Allopurinol with simultaneous application with Amoxisar increases the risk of skin rash.
Amoxisar reduces clearance and increases the toxicity of methotrexate.
Amoxisar with simultaneous application increases the absorption of digoxin.
