Dosage form: solution for external use

Pharmacological action: vasodilating.

Alopexy - Composition and form of release

Solution for external use 100 ml

minoxidil 2 g

auxiliary substances: ethyl alcohol - 55 ml; propylene glycol 30 ml; purified water - up to 100 ml

in a bottle of dark glass 60 ml (complete with a dispensing pipette); in the box 1 or 3 bottles.

Alopexy - Pharmacological action

Alopexy is a peripheral vasodilator that, when administered, has a vasodilating and hypotensive effect associated with the ability to open K + channels of smooth muscles. The predominant action is the expansion of arterioles and a weak effect on the veins, resulting in a pronounced reflex tachycardia and an increase in IOC; reduces pressure in the pulmonary capillaries.

Increases the activity of plasma renin and promotes the occurrence of secondary hyperaldosteronism (Na + and water retention). After a single application of the inside, the effect develops in 30 minutes, reaches a maximum in 2-3 hours and lasts 24-48 (up to 75) h.

With topical application stimulates hair growth in androgen-dependent alopecia. It improves microcirculation, stimulates the transition of hair cells to the active growing phase, changes the effect of androgens on hair follicles.

Reduces the formation of 5-alpha-dehydrosterone (possibly, indirectly), which plays an important role in the formation of alopecia. The best effect is for a small prescription of the disease (no more than 10 years), young patients, the bald spot in the top of the head is no more than 10 cm, and there are more than 100 gun and terminal hair in the center of the bald head.

The beginning of stimulation of hair growth begins not earlier than 4 months after the beginning of treatment. After the abolition of therapy, hair growth stops, and after 3-4 months. we can expect the restoration of the former appearance. Ineffective in case of baldness caused by taking drugs, improper nutrition (deficiency of Fe, vitamin A), as a result of hair styling in "tight" hairstyles.

With topical application of minoxidil, its stimulating effect on hair growth in people with androgenetic alopecia (hair thinning, baldness) was noted. With topical application, minoxidil stimulates the growth of keratinocytes in vitro and in vivo, as well as hair growth in some patients with androgenic alopecia.

This growth is observed after about 4 (or more) months of use of the drug and manifests itself differently in different people.

After discontinuation of therapy, repeated growth stops, and a return to the initial state occurs within 3-4 months.

The exact mechanism of action of the drug is not known. Local use of minoxidil in patients with normal and high blood pressure during controlled clinical trials did not cause systemic manifestations associated with the absorption of minoxidil.

Alopexy - Indications for use

артериальная гипертензия , в т.ч. Inside - arterial hypertension , incl. symptomatic (especially severe forms, resistant to combination therapy with antihypertensive drugs).

– андрогенная алопеция . Locally - androgenic alopecia .

Alopexy - Contraindications

Unambiguous contraindication

Hypersensitivity, chronic heart failure , not associated with hypertension, angina pectoris , exudative pericarditis , pheochromocytoma , chronic renal failure, mitral stenosis , secondary hypertension in the "small" circulation, pregnancy , lactation.

With caution

Violation of cerebral circulation (including stroke), myocardial infarction . Violation of the integrity of the skin, dermatosis of the scalp, elderly age - over 65 years (with local application).

Alopexy - Side effects

Decrease in blood pressure, orthostatic hypotension, headache , dizziness , fainting, tachycardia, arrhythmias, swelling (delay Na + and water in the body), dyspnea, hypertrichosis , thinning and strengthening of pigmentation of hair on the body, inversion of the T wave on the ECG.

In rare cases:

Hydrotorax , hydropericardium (in patients with CHF), angina pectoris (in patients with coronary insufficiency), paresthesia, allergic reactions (skin rash and itching).

With topical application:

Itching, dryness and flaking of the scalp; allergic reactions (dermatitis, urticaria , rhinitis, facial edema), hypertrichosis, seborrhea .

Alopexy - Administration and dosage

Inside, adults and children over 12 years of age, the initial daily dose - 5 mg, once, or in two divided doses. If the hypotensive effect is insufficient, every 3 days, the daily dose is increased by 5-10 mg. The average therapeutic dose is 20-40 mg / day (in one or more receptions); the usual maintenance dose is 0.25-1 mg / kg / day, the maximum dose is 100 mg / day.

Children under the age of 12 years - in the initial daily dose at the rate of 0.2 mg / kg in 1 or 2 admission. The average therapeutic dose is 0.2-1 mg / kg (not more than 50 mg / day). The maintenance dose is 0.25 mg / kg. With chronic renal failure, the dose should be reduced. Locally, 1 ml of 2% solution is applied to the scalp in the center of the focus of alopecia and intensively rubbed (from the center to the periphery) 2 times a day.

The duration of treatment on average is about 1 year.

Alopexy - Special instructions

Avoid contact with eyes. After application, wash hands thoroughly.
Accidental ingestion can cause serious systemic side effects associated with vasodilation (5 ml of the drug contain 100 mg of active substance, which is the maximum recommended dose of minoxidil).

Because of the possible development of severe tachycardia in patients with the treatment of hypertension, the drug should be combined with beta-blockers.

Before using the minoxidil locally, make sure that the scalp is healthy and in a normal state.

Increased absorption of minoxidil through the skin, which can cause systemic effects, can occur in the following cases:

- dermatosis or scalp lesions,

- the joint use of retinoic acid, anthralin or other irritating topical medication,

- increase in the dose and / or dosing frequency used: the recommended dosing regimen should be followed.

Similarly, although the systematic use of the minoxidil solution has not revealed any systemic effects, it can not be ruled out that the increased absorption caused by individual variability or atypical sensitivity can cause these systemic effects. It is necessary to inform patients about such a risk.

In case of systemic action of the drug (low blood pressure, tachycardia, signs of retention of water and salt, chest pain) or the occurrence of serious skin reactions, treatment should be discontinued.

The possibility of using the drug in patients with heart disease in history should be analyzed for the relationship between the benefits of treatment and potential risks. Such patients should be particularly clearly informed about possible adverse reactions, so that if they occur, they can immediately interrupt the use of the drug and contact the treating doctor.

Do not use Alopexy in the following cases:

- if there was a sudden loss of hair (complete baldness) after illness or therapy with any drug;

- with respect to any other part of the body.

Accidental swallowing can cause serious unwanted reactions.

In case of accidental contact with the eyes, the affected skin, mucous membrane, solution containing ethyl alcohol may cause irritation and burning sensations: rinse the affected area thoroughly with water.

While using minoxidil it is not recommended to be exposed to sunlight.

The drug contains propylene glycol and may cause skin irritation.

Features of the effect of the drug on the ability to drive a vehicle or potentially dangerous mechanisms: Not affected.
